After being injured on the job in front of terminal manager, dock supervisor and 2 other workers I went to Dr. that company had account with. at that time i was put on restrictions and was given physical therapy. This all started on November16th of 2022. I did all the things that the Dr. wanted went back and he told me that he wanted to take me off of my restrictions for 30 days then reevaluate my injury. That happened first week of 2023. gave paper work to my manager and went back to my normal duties on February 4th of 2023 at which time the Dr. put me on permanent restrictions. At this time my employer told me there was nothing i could do there so i should go home. My manager at that time told me "off the record" go to the dr. and have my restrictions lifted and i could come back.
So at that time I decided I could not do this alone and reached out to Mr. Bialke for legal council. Our first meeting at his office I told him all the details that I had and asked if this is something he could do. He said yes and told me of all my options and explained them to me so I understood them. He then told me that I'm the boss and he would move forward with my case. When I first met Dave I knew he was the legal council for me. He was straight to the point gave me multiple options to move forward along with real expectations. This is important to me because I had say on how things went. Dave is great at explaining all of the information and this is the big thing that i thought was important and gave me clear cut answers and made me decided on the direction we took. So basically he listens to you and your concerns explains all the options then you decided on which way to go. I asked a couple of times what would be the best way to proceed and he told me that it was my decision and he would follow that but if asked he will give you clear cut options to choose from. Dave Bialke was recommended to me by a high school friend. I was looking for help with a work comp claim that had been repeatedly denied by SFM Insurance. I had a bad fall on ice while picking up covid supplies from my school district central office in January 2021. I fell on my face, ribs, and left hand. I had damage to the left side of my face, needing 2 root canals and had chipped my front 2 teeth. I did get the root canals but SFM refused to replace my damaged front teeth, even though x-rays confirmed the damage from the fall.
Dave Bialke took my claim. He invited me to meet with him at his office and he gathered information. He made it clear that he would get paid from SFM as the cost to file would be included in the claim. He was very thorough, kind, and supportive.
After the claim was submitted, SFM approved the dental work to go forward right away. I couldn't believe how quickly he got this done, after 4 years of trying myself. I am so impressed and so appreciative of the amazing work he got done so expeditiously!
